Union Leave Bank Transfer Form
— The form can be emailed or mailed during the months of April and/or October only.
Expense Report Form
— Original receipts must be attached to completed expense reports for all expenses other than mileage. Original receipts can be digital, but only if that was the way the receipt was originally received. Original paper receipts must be turned in to the Treasurer to receive reimbursement. Digital may be sent via email.
— Completed reports can be brought to a meeting, mailed to the Union PO Box, or emailed to the Treasurer.
